Theinformation contained in the service record starts whenyou apply for enlistment and is added to throughoutyour naval service. The record is the property of theNavy. It must be safeguarded against loss and againstaccess by unauthorized persons. Only those personnelgiven the authority by the CO make service recordentries.The Enlisted Service Record is a folder thatcontains various forms concerning your enlistedservice. The right-hand side has various forms in aspecific order. (NOTE: Your service record will containonly the forms that apply to you.) There are 15 differentforms altogether. The order in which these forms arefiled has led to their being referred to as pages. Forexample, your enlisted contract is the first, orbottommost, form. It’s referred to as a Page 1.Other papers required for safe keeping or recordpurposes are filed on the left-hand side of the folder. Aseparator entitled Career Performance Data, NAVPERS1070/617, divides the left-hand side. Beneath thiss e p a r a t o r, a l l y o u r p e r f o r m a n c e eva l u a t i o n s ,commendations, and awards correspondence are filed.If you have a previous enlistment, a certified copy of theEnlisted Performance Record from the previousenlistment and copies of any Certificates of Release orDischarge from Active Duty, DD Form 214s, are alsofiled beneath the separator. All other papers are filedabove the separator in chronological order, the latestdate on top.Only three of the forms from the Enlisted ServiceRecord are covered in this chapter. They include the—Enlisted Qualifications History, NAVPERS1070/604,Dependency Application/Record of EmergencyData, NAVPERS 1070/602W, and theAdministrative Remarks Form, NAVPERS,1070/613The remaining forms are more or less of anadministrative nature.
